How do I book an appointment?

Please bring a form of identification (i.e. Drivers License or Passport). As well as insurance information if you have.

Will I receive my medication in the office?

Injections are administered in the office.

Other medication you will pick up at your local pharmacy once we call in your prescription.

How do I renew my prescription?
